Resistive lesions trapping follicular materials with myraids of plugged follicles
Grade III :
Inflamed acne papules and pustules with other milder stages of non-inflamed lesions
Grade IV :
Inflammatory acne lesions, open comedones, closed comedones, papules, pustules, nodules and cysts
